Surest Copay Only Plan (All employees except Hawaii)
You asked for a new lower-cost medical plan and we delivered! Say hello to the Surest Copay Only Plan, with no coinsurance or deductibles and lower costs per pay period. With this Copay Only plan, you’ll know the cost of a medical service and can compare prices among providers.
This plan is administered by UnitedHealthcare and provides in-network coverage only through the same broad provider network offered by UnitedHealthcare today. It removes deductibles and coinsurance entirely and results in a clear and upfront copay for each service. Depending on the service you search for, you’ll have a variety of providers with fixed copay amounts that you can select from. The highest quality providers yield the lowest copay amounts, bundled into one cost for any service — even for complicated surgeries — which helps you avoid unexpected bills in the future. Cost per pay period is changing
Worried about cost? We’ve got you, Team Magenta! T-Mobile will continue to cover the majority of the cost to keep employees’ cost increases as low as possible. The amount we contribute will be the same no matter which plan you choose. T-Mobile won’t steer you toward any plan – it’s all about you choosing what’s best for YOU.
Your cost per paycheck will change in 2025. How much it changes depends on what plan you select. For some plans, the cost is going up, and for others it is going down! Review the cost impact to your plan by logging in to the enrollment tool at t-mobilebenefits.com.
Vision plan costs
Small rate increase—big savings on your exam copay! Vision costs per paycheck will increase by a small amount per pay period; however, your coverage remains the same and the exam copay drops from $15 to just $5! Learn more.
Enhanced Paid Family Leave & Short-Term Disability
Should you need to take time off from work to care for yourself or a loved one, we have options to support you.
Beginning January 1, 2025:
- We’re enhancing our Short-Term Disability and Paid Family Leave benefits to support you at times when family or other needs require you to step away from work for an extended period of time. T-Mobile has your back.
- Grandparents, you’re in: We are expanding the Paid Family Leave benefit to include caregiving for grandparents, and grandparents will also be eligible for Paid Bonding Leave with a grandchild. Learn more.
Expanded family building benefits
Beginning in 2025, we’re expanding our family building benefit offerings with Maternity Support Services, including Progyny’s new Pregnancy and Postpartum support program that offers resources, support, and education from labor and delivery nurses, doulas, mental health professionals, and lactation/feeding experts.
We’re also introducing a new doula benefit covering up to $2,000 per birth for birthing and postpartum doula care, adding coverage for tubal ligation reversals and vasectomy reversals, and increasing the Adoption and Surrogacy reimbursement from $14,000 to $30,000 per child/surrogacy. Learn more.
